- For planned absences of LESS THAN 5 days, please complete the ‘Request for Leave’ form below.
- For extended planned absences, MORE THAN 5 days please download and complete an ‘Application for Extended Leave – Vacation & Travel’ form and forward to principal@plcarmidale.nsw.edu.au
